How To Get Services Started:
Pediatrics Unlimited, Inc. accepts referrals from parents, doctors, and other agencies serving children in the upstate of South Carolina. In order to get started with therapy services, please contact either our Spartanburg clinic at 864-595-4225 or our Greenville clinic at 864-627-0009. One of our office managers will collect some basic information from you. We will also need:
*A Prescription of Referral from your child's primary care physician. For
For parents requesting services, please fill out the Referral for Service
form and send it to either our Spartanburg clinic or our Greenville clinic.
*A copy of your primary insurance card and/or your child's Medicaid card.
*Completed and signed New Patient Forms.
*If your child is receiving a Feeding Evaluation, please also fill out either
the Feeding History for Infants/Toddlers or, for an older child, the
Feeding History for Children.
What To Expect On Your First Visit:
The first appointment with one of our therapists will include an evaluation,
consultation regarding the test results, and initiation of treatment when applicable. Please plan to arrive at least 15 minutes before your scheduled appointment time in order to fill out necessary paperwork. Evaluations typically last 60 to 75 minutes.
